GFAP (glial fibrillary acidic protein) is an intermediate filament protein expressed by numerous cell types of the central nervous system. In the central nervous system, GFAP is expressed by astrocytes and ependymal cells, but not by other glial cells. However, immature oligodendrocytes and immature choroidal plexus cells can be GFAP-positive. In the peripheral nervous system, enteric Schwann cells and satellite cells of the human sensory ganglia express GFAP. Outside the nervous system, GFAP is found in myoepithelial cells and chondroblasts, the former coexpressing with cytokeratin and the latter with vimentin. Astrocytoma, ependymoma, glioblastoma, and oligodendroglioma are almost always positive. Plexus carcinomas, gangliogliomas, and primitive neuroectodermal tumors (PNET: neuroblastoma, among others) express GFAP to varying degrees. Schwannomas and neurofibromas frequently express GFAP. Chondromas, chondrosarcomas, and pleomorphic adenomas are also GFAP-positive in most cases. A few carcinomas (primarily from the lung and breast) can express GFAP. GFAP can be detected in sustentacular cells in paragangliomas. This marker is primarily used to differentiate neoplasms of astrocytic origin from other neoplasms of the central nervous system.
