Immunoglobulin G is the most abundant antibody isotype in the circulation. Human IgG4, one of the four subclasses of IgG, contains a gamma-4 heavy chain and a hinge region that is shorter than that of IgG1. Its two primary effector functions are complement activation and binding to the Fc-gamma receptor of effector cells to initiate phagocytosis. Human IgG4 accounts for less than 6% of total serum IgG levels. IgG4 is overexpressed in inflammatory pseudotumors (IPT) and underexpressed in inflammatory myofibroblastic tumors (IMT). In pulmonary nodular lymphocytic hyperplasia (PNLH), there is an increased number of IgG4+ plasma cells.
