TdT (terminal deoxynucleotidyltransferase) is a specialized DNA polymerase that adds N-nucleotides to the V, D, and J exons during antibody gene recombination, thus enabling junctional diversity. It is expressed in immature, pre-B, and pre-T lymphoid cells, as well as in acute lymphoblastic leukemia/lymphoma (ALL) cells, but not in ALL pre-B cells or normal mature T or B lymphocytes. Anti-TdT is positive in approximately one-third of all cases of chronic myeloid leukemia, making it a good indicator of improved response to chemotherapy. TdT is also found in malignant tumors of lymphoid origin and in thymomas. It is a sensitive and specific marker for lymphoblastic lymphomas/leukemias.
