Langerin/CD207 is a membrane-associated type II receptor expressed exclusively by Langerhans cells, in astrocytomas, and malignant ependymomas, but not in normal brain tissue. It recognizes mannose residues, induces membrane overlap and zippering, leading to the formation of Birbeck granules. Tumors derived from Langerhans cells are divided into Langerhans cell histiocytosis (LCH) and Langerhans cell sarcoma (LCS). LCS is a rare dendritic cell tumor defined as a malignant high-grade variant of LCH. Differentiating between LCH, LCS, and other tumors is difficult. The presence of Birbeck granules, CD1a, and expression of Langerin proteins are helpful in distinguishing Langerhans cell diseases from other non-Langerhans cell proliferations.
