The S100 protein can be found in cell membranes, cytoplasm, and nuclei. The protein is a dimer consisting of two subunits, alpha and beta, with extensive sequence homology. There are three forms of the S100 protein: alpha-alpha, known as S100A0; alpha-beta, known as S100A; and beta-beta, known as S100B. Since most cells containing S100 protein also express the beta chain (notable exceptions are neurons and histiocytes), this has become almost synonymous with the protein. Its properties are related to many basic cellular functions, such as cation diffusion across lipid membranes, microtubule assembly, and RNA polymerase activity. S100 (beta protein) is present in glial cells, Schwann cells, and satellite cells (but not in perineurial cells), melanocytes, myoepithelial cells, some glandular epithelia (breast, kidney), skeletal and cardiac muscle cells, fat cells, and chondrocytes, as well as follicular dendritic cells. S100 is a very sensitive marker for all types of malignant melanoma; negative staining is extremely rare. Due to its low specificity, other markers should be included in a panel for malignant melanoma, such as PRAME and Melan-A. S100 can be used in the differential diagnosis of sarcomas (e.g., to distinguish between liposarcomas and other myxoid tumors) and spindle cell tumors (e.g., to distinguish between schwannoma, leiomyoma, and gastrointestinal stromal tumor).
